If during your Physionutrition consultation, your health professional has asked you to fill in ‘online questionnaires’, you just need to click on the evaluation space at the top right of the screen to access them.
To begin, you will be asked for your login and password. These will have been given to you by your health professional.
Your results will be sent directly to your health professional, who will analyse them and propose a treatment tailored to your needs.
